You have one male and one female Sim relax on the same bed (or be in the same hot tub or clothing booth). Then select one Sim and click on the other. Pick the option "Try for Baby", which will appear if their relationship is high enough. If the female Sim becomes pregnant, you will hear "Rock a Bye Baby" chime afterwards. If not, then you will need to try again another time.

Or, you could call the Adoption Service on the phone. You are guaranteed a baby,toddler, or child that way.
