Okay, I'll try to list them all, but I might miss a few:

1925-Starring Lon Chaney
1943-Starring Claude Rains
1962-Starring Herbert Lom
1987 cartoon
1989-Starring Robert Englund
1990-Starring Charles Dance
1998-Starring Julian Sands
2004-Strring Gerard Butler and Emmy Rossum

Then there are a few of them that are loosely based and have different names.

Song at Midnight (1937)
Phantom of Paradise (1974)
The Phantom of Hollywood (1974)

Um, I think that's all of them...

You know what, here's a list: link)

Adding to the list:
1991 David Staller proshot non ALW play
2009 Angel of Music
1983 Maxamillion Schell, another made for tv thing currently unavailable in NTSC as far as I can tell.

Ok, so for the sake of being INSANELY comprehensive, there are also several...well...someone actually made Phantom porn. Like more than one. The ones that I have actually SEEN (almost ashamed to admit that one) were all made in 1989 and had the same kind of weird Italian looking dude playing the Phantom. They are as follows:
Phantom X- More or less tried to keep some semblance of the original plot...ish. Phantom named Erik, everyone else got new monikers. Oh yeah, and at the very very end the Phantom gets unmasked (after being shot) and it turns out he wasn't deformed after all. What a cop out. But it's fun to laugh at the attempt at period costume. (well, when they're actually wearing clothes)
Phantom of the Cabaret part 1 & 2- Ok, so I've only seen this really randomass clip from part 2. But apparently this thing was filmed on location in Paris. Big whoop. Not like you're seeing anything but about 3 half assed sets. Yeah the pic with the really godawful half mask? That's from this. This one has like nothing to do with the original plot. The closest thing you get is this chump in a half mask, who has a massive thing for this showgirl chick, and used to play the violin back in the day. So this one is a laugh and a half because toward the end you get to see the 'deformity'. I had to pause. Squint. Tilt my head a lil bit. Lean in...Oh, wait! THERE it is! Yeah, makes Gerik's sunburn look like Leroux's full noseless corpsey glory.
And the sad thing is, from what I've heard, these are the GOOD ones! At the very least they're funny. I mean, I laughed hysterically through them. The bad acting is PRICELESS. The chick in Phantom X makes Emmy look like frigging Ingrid Bergman.

Yes, I used to have it. It disturbs me to no end that there was a Phantom movie involving Pauly Shore.
