I think Malcolm in the middle was the middle kid and he's the one telling the story. He talks directly to the viewers through the camera.

But The Middle is about a middle-class Mom and her family's life. She's just the narrator but she doesn't look at the camera or anything.

I think they are both just comedies about that person's life and what they go through with the family.
